I can't recommend a a specific book, though, since I haven't read any of them 
:).

Regarding the level and type of "good" documents / tutorials... well, I guess 
it depends on the personal aspect and taste. An experienced C/C++ programmer 
would likely prefer an API reference (that's the most useful source of 
information I use with regard to Qt: http://qt-project.org/doc/qt-4.8/), a 
beginner might want to be 'guided' perhaps. But what is good guidance?
