No, it's not necessary to run every Qt application. Just the ones that want to 
display any GUI on Windows.

If you're ok with a text mode (console) application, you don't need this 
plugin.

Officially? yes. But it might be broken. It's not tested.

No solution is known yet. If you try and find out a way, post it to the bug 
report you linked to. Please try modifying ANGLE sources too.
