[Qt-creator] Windows did not appear when running

Hi,

I have a simple Qt program with a dialog box, holding Label and LineEdit objects. But when I run the program from the QtCreator the window did not appear and the program ends with the return staus 0.

I give the source code and the .pro in the attached files.

can somebody help me out.
