We use the commercial version of both VC8 and VC9 and do not have any problems with having to rebuild. If you could be more specific as Oliver suggested, I may be able to help.

What problems are you facing exactly? The project doesn't link after an incremental build? Classes you have modified are not properly re-compiled? Some mock'ing/uic'ing steps are not done (on modified *.h/*.ui files)?
Well, to be honest, I would not have an answer on any response you would give, but maybe other people could help then better.
I would not see why not being able to incrementally build a project should be dependend on some used library (Qt 4.5.2 in this case) or what Qt 4.5.2 specific header files would cause Visual Studio to fail in doing an incremental build. But maybe if you give us the answer what *exactly* fails it will become obvious.
Cheers, Oliver
