The document on
http://www.digitalfanatics.org/projects/qt_tutorial/chapter11.html was
written for a Qwt 4.x and is completely outdated.

If you insist on this example you have to install Qt 3.x + Qwt 4.x. As the
document was written for Qwt 0.4.1 ( == 4.1.0 ) you might need an early Qt
3.x and Qwt 4.x version. 

Note that Qwt 0.4.1 is from 2002, so don't expect any support for this
environment.

PS: better use the official Qwt support channels. I don't read this list on
a regular base.
