You might want to refer to this message I recently posted:

It tells you how your application will find libraries (NOT plugins) on which (or its PLUGINS) depend (on Windows, Linux, Mac ;).

Note again that the Qt MySQL plugin might depend (very much likely) on external libraries (again: 'ldd' command is your friend here!), provided by MySQL. Make sure that these libraries are accessible to your application, e.g. by following the suggestions in above message.
