I met to say VSIntegration hasn't been worked on since 4.3 or 4.4 timeframe, 
I'm surprised it ever worked with 4.5.1.  It broke for me when I moved from 
4.4 to 4.5.x (don't remember what x is) in exactly the same way as you.  I 
didn't find it too objectionable to edit the .ui file to remove the first 
line <?xml>.... it seemed pretty compatible beside that.  But then I bit the 
bullet and switched to the add-in.
