[Qt-interest] Customizing the Standard Context Menu

I read the class documentation on how to add my own menu to the standard
context menu. But I am in interested in doing something a bit different
than what is suggested. Is there a way to do any of the following:

1) Add my own menu at the _start_ of the standard menu instead of at the
   end?

2) Customize what is actually displayed on the standard menu so that I
   can leave off the unicode option?
