How exactly do you measure the size in "preview"? Do you display the PDF at 100% and then measure the font sizes on screen, using a ruler?
I am using a Mac myself, but I am not sure whether "Preview" and other Mac apps properly respect the DPI settings of your screen, not to mention whether the screen DPI settings are actually set correctly.
You could verify the screen DPI/app settings by displaying an "a4 PDF" at 100% and then put a "real" a4 paper in front of your screen! If those match, then chances are very high that Mac OS has properly detected and set the display DPI settings AND the "preview" app properly respects those.
Now I would expect that this is the case, but just to make sure ;)
