[Qt-creator] help

I've been running Qt-Creator 0.9.1, and now 0.9.2 for about a week now.  It
has operated quite well for a pre-release version.  I have had several
instances where the application suddenly stopped and disappeared when I was
editing the GUI  (*.ui file).  However, my biggest problem now is that I
can't seem to get the debugger to work at all.  When I press the start
debugger button (or press F5), the program compiles (compile bar turns
green), but then nothing happens.  I've waited for over 20 minutes at times
just to see if it would eventually start, but nothing happens.  I end up
having to press the red STOP square.  

 

Any ideas?

 

Do I need a startup script for gdb?

 

 

Occasionally, while I'm trying to set breakpoints (while the debugger is
"running"), I get this error dialog:
