I can recommend the OpenGL Super Bible. Again it doesn't use Qt directly but 
to be honest that is no problem. Porting the examples to QGLWidget is trivial.

I have the 4th edition which covers the legacy fixed function pipe-line and 
then moves onto the new shader-based pipe-line. I think the new 5th edition 
dives straight into the shader-based way of things since the legacy is stuff 
is being deprecated.

Well worth the investment.
