3 Questions about localizing qml-files

1) Qml-files seem to get ignored in the lupdate command. Adding qml to
the extensions list does not seem to work

2) identical text which needs to be translated differently can be
tagged with the tr("to be translated", "note to the translator")-Syntax
...and this works well. However, if you use a phrasebook for
retranslation from scratch via batch-translate  then all occurences
(tagged or not) will be translated the same

3) I have not managed to get several layers of translation working with
qml-files. 
Use case: I have a translation for a specific locale, but a user has
special requirements (i.e. on his machine alone some translations need
to be different).  How do I manage it that his translations supplant the
translations in the standard qm file for his locale?
