[Development] Module maintainer's explicit approval required for Qt 5 inclusion

Now that I've said that, the release team wishes to release the following 
modules, so we're looking for an explicit ok from someone who can say so:

qt3d
qtactiveqt
qtimageformats
qtgraphicaleffects
