Qt 5 had High-DPI support for OSX since a few minor versions. Experimental High-DPI support for Windows and Linux have recently been added in 5.4. Here is an overview of the support:

Legacy widget based applications should get some High-DPI improvement by just using Qt 5.4. Depending on what the applications do, some adjustment will be needed, though. Especially the inclusion of high-resolution graphics/icons for the High-DPI case.
